The Comprehensive Benefits of Dual SGLT Inhibition:Perspective of Cardiologist
Abstract
Heart failure (HF) represents one of the earliest and most burdensome cardiovascular complications in patients with type 2 diabetes mellitus (T2DM). Despite advances in glucose-lowering therapies, the risk of HF remains high, underscoring the need for preventive interventions. Canagliflozin, a sodium–glucose cotransporter 2 (SGLT2) inhibitor, has emerged as a key therapeutic agent with cardioprotective effects beyond glycemic control. Evidence from large-scale clinical trials, including the CANVAS Program and CREDENCE study, demonstrated that Canagliflozin significantly reduces hospitalization for heart failure and provides benefits even in individuals without prior HF. The mechanisms of protection are thought to involve osmotic diuresis and natriuresis, leading to reduced plasma volume and lower cardiac preload and afterload, as well as improvements in myocardial energetics, endothelial function, and renal hemodynamics. These combined effects contribute to better cardiovascular stability and early prevention of HF in diabetic patients. In clinical practice, Canagliflozin thus represents a valuable option for proactive management of T2DM patients at risk for HF, reinforcing its role in integrated cardiometabolic care and shifting the paradigm from treatment of established disease to prevention of first events.

Abstract
From an endocrinologist’s standpoint, optimal diabetes management extends beyond HbA1c reduction to encompass weight control, metabolic balance, and long-term organ protection. Canagliflozin, as a dual SGLT2 and SGLT1 inhibitor, offers a unique mechanism by reducing both renal glucose reabsorption and intestinal glucose absorption. This dual action results in improved glycemic control, reduced postprandial glucose variability, and meaningful weight loss. Additionally, it contributes to reductions in blood pressure and uric acid levels. Evidence from clinical trials has demonstrated sustained HbA1c lowering alongside metabolic benefits without increasing hypoglycemia risk when used appropriately. This session will highlight how dual SGLT inhibition addresses multiple pathophysiological defects in type 2 diabetes and supports a more holistic, patient-centered treatment strategy.

Abstract
From a nephrologist’s perspective, slowing chronic kidney disease progression is a critical priority in patients with type 2 diabetes. Canagliflozin has demonstrated robust renal protective effects, as evidenced in the CREDENCE trial, showing significant reductions in kidney failure, albuminuria progression, and renal-related outcomes. Through mechanisms including reduction of intraglomerular pressure via tubuloglomerular feedback, decreased hyperfiltration, and anti-inflammatory effects, SGLT2 inhibition plays a key role in nephroprotection. The additional SGLT1 inhibition may further contribute by improving glycemic variability and reducing metabolic stress. Importantly, canagliflozin provides benefits independent of glycemic control, reinforcing its role as a disease-modifying therapy. This session will focus on integrating dual SGLT inhibition into CKD management to achieve comprehensive renal and systemic protection.
